Our Hospice at Home Care service aims to provide you with overnight care when you are within the last four weeks of your life.
This service started in August 2010 and now receives referrals from all GP surgeries in Corby. Also, any Lakelands patient who has used our Day Services in the earlier stages of their illness, will also be able to have the hospice at home service. The service is available 365 days a year.
Hospice at Home Nurses will work very closely with your GP and District Nurses and share the care that needs to be given over a twenty-four-hour period. This will allow you choice about where you wish to be cared for and may avoid hospital admission. You must be over 18 years of age.
All Lakelands Hospice at Home Nursing staff:
Lynn Davies our Hospice at Home Co-Coordinator said “Patients will be referred to the Hospice at Home service in the last four weeks of life. Referrals will come from district nurses. The service will run from 10pm until 6.30am liaising with the district nurses during the day, to attend the patient overnight and then handing back to the district nurses the next morning. The service will be operated by a specialist team of qualified nurses. In order to maintain continuity for the patient wherever possible the same nurse will attend the patient each night.
Tracy Glen (Lakelands Nurse Manager) says “How people die remains in the memory of those who live on and it is important to get this right. Our service aims to give patients choice and for us to make that choice become a reality”.
