Hospice at Home Care

Hospice at Home Care is a new service providing overnight care for terminally ill patients over the age of eighteen who live in Corby. The professional care provided by our nurses will enable people who choose to stay at home achieve the best quality of life. This service will start from Lakeside Practice and expand surgery by surgery.  It is hoped that Lakelands patients who have used the Day Care Hospice in the earlier stage of their illness will also now be able to have more intensive nursing care at the end of their illness. The service will be available 365 days a year. 

Hospice at Home Nurses will work very closely with patient’s G.P’s and District Nurses and share the care that needs to be given over a twenty four hour period. This will allow patients choice about where they wish to be cared for and may avoid hospital admission. Currently Cynthia Spencer Hospice provides a Hospice at Home Service for Northamptonshire but our service will complement that service specifically for the people living within the Corby area. 

All Lakelands Hospice at Home (LH@H) Nursing staff: 
  • Are Registered Nurses with additional training in palliative care.
  • Work in accordance with the Nursing and Midwifery Council (NMC).
  • Will provide support for patient, carer and family member’s throughout our intervention.
  • Will always respect the personal property of patient’s and treat it with due care at all times.
  • Maintain Lakelands Health and Safety Policies.
  • Will undertake a risk assessment for every patient and will ensure records are maintained should an accident or incident occur.

Pat Clipston Hospice Home Co-ordinator saidPatients will be referred to the Hospice at Home service in the last two weeks of life.  Referrals will come from district nurses.  The service will run from 10pm until 6.30 am liaising with the district nurses during the day, to attend the patient overnight and then handing back to the district nurses the next morning.  The service will be operated by a specialist team of qualified bank nurses.  In order to maintain continuity for the patient wherever possible the same nurse will attend the patient each night.  An on-call doctor will be available.”
