Hospice at Home

Our Hospice at Home Care service aims to provide you with overnight care when you are within the last two weeks of your life. The professional care you would receive from our nurses will enable you to stay at home to maintain the best quality of life and dignity.
Day Care Hospice
Living with Heart Failure / COPDThe Living With Heart Failure / COPD (Chronic Obstructive Pulmonary Disease) Programme is a 16-week programme in which you will receive a wide range of information to help support you in living with your condition.

Referrals & ContactsIf you would like to be referred to our Day Care Hospice or you are the carer or a family member of someone who you would like to refer on their behalf, then please either download our services_ leaflet.pdf or give us a call (01536) 747755 Mon-Fri 9-5pm. We look forward to hearing from you.
Therapies & ActivitiesThere is a variety of facilities and activities available to you as a patient at our Day Care Hospice. Unless stated, everything is free of charge to you. Some may require advance booking, our nurses and volunteers will be more than willing to help you with arranging the things you want to do.

Our skilled nursing team and volunteers are here to help you live your life as fully as possible by providing physical, emotional and spiritual support in a welcoming and compassionate social environment. The day care hospice is open to anyone living within 20 miles of Lakelands Hospice and living with a life limiting illness.
